Horizons Trust enables adults of all ages to develop confidence, build relationships, and learn life skills through a range of diverse activities, in a fun, safe, caring and supportive environment. We do this by preparing plans that are tailored to the individual's personal needs and goals, in conjunction with their family or support person. Delivering activities that best achieve the outcomes promoted in the individual plans with regard to delivering opportunities for learning and social participation.
